Developmental Services

Seven Counties Services believes that everyone, regardless of how severe their developmental or intellectual disability may be, has the right to live independently. With support from birth to senior years, our staff can help families create a great home environment for a child or adult.

Services are available in Bullitt, Henry, Jefferson, Oldham, Shelby, Spencer and Trimble counties.

DEVELOPMENTAL SERVICES — 502.459.5292

Common Ground Day Training services support participation of consumers in daily, meaningful routines both on-site and within the community. Day training services stress training in activities of daily living, self-advocacy, adaptive and social skills which are age and culturally appropriate. 502.451.1148

Crisis Prevention and Response provides on-site prevention and response services to individuals, birth through the life span, who are diagnosed with developmental disabilities. These services are to assist the individual through a crisis, while helping them maintain placement in the least restrictive community environment. These services include, but are not limited to, the following: assessment, support services, on-site crisis response and respite.

Family Supports Services is the regional Point of Entry for accessing available supports and services for persons with developmental disabilities. This includes providing direct services and/or coordinating access to needed services to individuals in our community.

PASRR Assessment is a federally mandated program designed to ensure that all applicants to Medicaid-certified nursing facilities are screened for evidence of a possible severe mental illness or a developmental disability. PASRR helps to determine if persons require nursing facility level of care, is the least restrictive in nature, and if specialized services are needed. The goal of PASRR is for the applicant to receive the individualized services they need, in the least restrictive environment; often this being a community placement.

PASRR Specialized Services provides individualized supports for people who reside in a nursing facility. The services include continuous, aggressive specialized training to prevent the regression or loss of current optimal functioning and to promote the acquisition of skills necessary for the individual to function with as much self-determination and independence as possible. 502.459.5292

Waiver Services provide supports and assistance to persons with developmental disabilities to maximize their independence, self-advocacy, and interaction with our community. The service array varies by individual waiver, but most waivers offer services such as Case Management, Behavior Supports, Supported Employment, Day Training, etc. Waiver Programs include:

Supports for Community Living (SCL) provides an array of services supporting alternatives to institutional care for individuals with Developmental and/or Intellectual Disabilities.

Michelle P. Waiver (MPW) is a program providing an array of services as an alternative to institutional care for individuals with developmental disabilities. MPW allows individuals to remain in their homes with services and supports.  Click here for a current list of Michelle P. Waiver providers

Acquired Brain Injury (ABI) provides services and supports to adults with acquired brain injuries who are working to re-enter community life. Services are provided in community settings and focus on obtaining and monitoring needed services.

Work Matters offers supported employment services which include: person-centered job planning, individualized job placement, skills training and on-going supports. Consumers with or without waiver resources may receive these services. Job seekers are assisted to secure and maintain both customized and labor market positions in an integrated setting.
